“Good luck, Mr. Evans!”
Mr. Tony Evans was admitted to Hallmark Healthcare after being hospitalized for hypertension and complaints of leg pain. Mr. Evans had a history of a stroke leading to right side hemiplegia. Over the months, he complained of increasing leg weakness and increasing difficulty to take care of himself. He received therapy in the hospital and arrived at the facility requiring moderate assistance for transfers, ambulation, and unable to complete steps. He was also experiencing a decreased cognitive ability.
After receiving physical, occupational, and speech therapy, Mr. Evans was performing all activities of daily living with modified independence, walking with a rolling walker and stairs with handrails while requiring stand by assistance, and improved cognitive abilities. After discharging from Hallmark, Mr. Evans returned home with family and home health services to continue his progression!
